Tesofensine Side Effects
Tesofensine is a triple monoamine reuptake inhibitor (serotonin, dopamine, noradrenaline) studied for obesity treatment. It has a more significant side effect profile than most peptides and requires close physician monitoring.

Side effects to be aware of
Dry mouth
CommonVery commonly reported, affecting the majority of patients. Staying well hydrated and using sugar-free gum or lozenges helps manage this.
Constipation
CommonA common side effect. High fibre intake and adequate hydration help significantly. Your physician may recommend stool softeners if needed.
Increased heart rate
Requires monitoringTesofensine can raise resting heart rate. This is monitored at each clinical visit and dose adjustment is made if the elevation is meaningful.
Insomnia or sleep disturbances
CommonThe stimulant-like properties of tesofensine can disrupt sleep, particularly if dosed late in the day. Morning dosing is preferred.
Anxiety or agitation
UncommonSome patients experience mild anxiety or restlessness, particularly early in treatment. Dose reduction usually resolves this.
Blood pressure increase
Requires monitoringNoradrenergic activity can raise blood pressure. Blood pressure monitoring is part of the supervised protocol.
Decreased appetite (intended effect)
CommonSignificant appetite suppression is central to the mechanism and is expected.
Monitoring
Blood pressure and heart rate monitoring at every visit is required. Psychiatric assessment before starting is recommended.
Who is a good candidate?
- Adults with obesity or significant overweight who have not responded adequately to other approaches
- Those without cardiovascular contraindications who are monitored closely by a physician
- Patients with cardiovascular disease or hypertension
- Those taking MAOIs
- Patients with anxiety disorders or psychiatric conditions without specialist clearance
Contraindications
Your physician screens for these conditions before prescribing. Do not attempt to self-prescribe.
- Uncontrolled hypertension
- Cardiovascular disease or recent cardiac events
- Monoamine oxidase inhibitor (MAOI) use or within 14 days of stopping an MAOI
- History of psychiatric disorders including psychosis, mania, or severe anxiety
- Pregnancy and breastfeeding
- Hyperthyroidism
When to contact your physician
- Nausea, vomiting, or digestive symptoms that are severe or do not ease within two weeks
- Any chest pain, palpitations, or significant change in heart rate
- Signs of allergic reaction: rash, hives, swelling of the face or throat, or difficulty breathing
- Injection site reactions that worsen, spread, or do not resolve within 72 hours
- Any new or worsening symptoms that concern you, regardless of whether they seem related to your protocol
- Changes in any monitored values outside the range your physician specified
